Financial and Insurance Information

 

Here at Digital Dental Arts, we believe that everyone deserves a healthy and beautiful smile, which is why we work hard to make sure every patient is able to optimize his or her insurance benefits. We also believe in transparency: we make sure that our patients understand the financial costs of their treatment before receiving it.

Here is a breakdown of our financial policies:

Pre-treatment estimates

We will inform you of the cost of all services before taking any action, in order to allow you the maximum control over your medical and financial situation. As long as you are able to provide us with the appropriate insurance information, we will work with your insurance company to provide estimates of your out-of- pocket cost as well.

Insurance Policy

As mentioned above, we will do everything in our power to work with your insurance company in order to provide estimates and help you maximize your benefits. However, it is important to understand that insurance policies change very frequently, and it is ultimately your responsibility to understand your insurance policy. If you have any doubts, we recommend speaking with your insurance company beforehand in order to avoid any potential problems. If, for any reason, you insurance company fails to make a payment within 45 days, you will be responsible for paying any remaining balances.

Payment options

Payment is expected at the time of service. In order to make this more convenient for our patients, we offer several payment options:

  • Cash
  • Checks
  • Master Card, Visa, and American Express

In the case that your insurance company will be paying a portion of the cost, you will only be asked to pay the out-of- pocked estimate at the time of service. However, it is important to remember that the insurance estimate at the time of service is only an estimate. Should your insurance company leave a remaining balance, we will notify you, and that amount will be due upon notification.

Cancelation Policy

If, for any reason, you must cancel an appointment, we ask that you give at least 24 hours’ notification. In the case of a missed appointment, we reserve the right to charge your account a missed appointment fee of $100.00.
